Program of Assertive Community Treatment (PACT)

We understand that traditional outpatient care doesn’t meet everyone’s needs. Some individuals benefit from more frequent, personalized support to manage their mental health and work toward greater stability and independence.

PACT provides intensive, long-term mental health services for individuals living with mental health disorders who need additional support beyond traditional outpatient treatment.

Rather than expecting individuals to navigate multiple services on their own, our team works collaboratively to bring coordinated care into the community.

Comprehensive, Team-Based Care

PACT brings together professionals from multiple disciplines to provide comprehensive, individualized support. Our team includes a medication management prescriber, nurses, therapists, a peer support specialist, a substance use provider, a case manager, and a program coordinator.

Together, these professionals develop a coordinated approach to care that reflects each individual’s needs, experiences, and treatment goals. By working as one team, we can address mental health concerns alongside other challenges that may affect a person’s daily life.

Our goal is to provide consistent, meaningful support while helping individuals develop skills, strengthen their connections, and reduce barriers that may have made it difficult to remain engaged in treatment in the past.

Meeting You Where You Are

One of the defining features of PACT is that services are provided primarily in the community, rather than relying solely on traditional office appointments.

Our team meets with individuals in their homes or other community locations where they feel comfortable. This approach allows us to provide support in the environments where people live, work, and navigate everyday challenges.

Individuals participating in PACT typically meet with members of the team approximately three times per week or more, depending on their needs and treatment goals. These regular connections allow our team to provide ongoing support, respond to changing circumstances, and work collaboratively with each individual toward meaningful progress.

A Long-Term Commitment to Recovery

PACT is a voluntary program that emphasizes consistent engagement and long-term support. Individuals who participate are asked to commit to at least one year of services, recognizing that meaningful change and recovery often take time.

Throughout treatment, our team works alongside each person to identify their goals, address obstacles, and build the skills and support systems needed for greater stability and independence.

Recovery looks different for everyone. Our role is to provide the ongoing care, encouragement, and resources that help individuals pursue the life they want while reducing barriers that may have interfered with treatment in the past.
